Repeat Prescriptions
Patients can request repeat prescriptions in the followings ways:
- In person - using the white tear-off request slip from your previous repeat prescription will help ensure all your details are correct.
- By email to - reception.pontprennau@wales.nhs.uk please use Prescription Request in the subject header and ensure that your name, address and date of birth are included in the main body of the email and the medication you require is clearly indicated
- NHS App - Please register for the app following the link https://app.nhs.wales/login
*Please note that we do not accept prescription requests over the phone.
*Please allow 72 hours when ordering your repeat prescription.
MONDAY prescription request will be ready for collection on THURSDAY
TUESDAY prescription request will be ready for collection on FRIDAY
WEDNESDAY prescription request will be ready for collection on MONDAY
THURSDAY prescription request will be ready for collection on TUESDAY
FRIDAY prescription request will be ready for collection on WEDNESDAY
Please also be aware that our local pharmacies need time to process your prescriptions as well.
Hospital Requests
When you are discharged from hospital you should normally receive 7 days’ supply of medication.
The hospital will issue you with your new medication requirements. Please bring this to the surgery or post via a stamped addressed envelope before your supply of medication has run out.
Hospital requests for change of medication will be checked by the GP first and, if necessary, your Doctor will issue you with a prescription. The Practice will endeavour to issue you with your prescription on that day, but it cannot be issued until your medical details are checked by the Doctor. Your prescription should normally be ready by 4pm on that day, or you may be advised to attend the next day.
The Doctors will review your medication regularly, which may involve changes to your tablets in accordance with current health Board policies. Please be reassured that this will not affect your treatment.
Medication Reviews
Patients on repeat medication will be asked to see a doctor, pharmacist or practice nurse at least once a year to review these regular medications. A notification should appear on your repeat slip.
Please ensure that you book an appropriate appointment to avoid unnecessary delays to further prescriptions.
