There is a new patient self-referral form to access abortion care throughout Cardiff and Vale. This is to replace the previous phone line, although that number is still available to call if someone is unable to complete an online form.
All information about the abortion service is available on the above website, but the below is worth noting:
- To avoid delays in getting an appointment, it is important that you fill all demographics correctly on the form -- incorrect details often mean we cannot find you/or create a new patient on the system to generate an appointment.
- It can take up to 3 working days for us to get back to you, but we will get back to you with an appointment date.
- We usually send an appointment by text, if we need to call it will be on a withheld number.
X-Rays
After seeing the GP you may be referred for an x-ray. The GP will send the referral to the x-ray department but you will then need to contact the x-ray department to book your appointment. The contact number you need is 02920 744564.
Urine Samples
If you have been asked by a member of staff to drop a urine sample to the surgery please bring the sample to surgery by 12 o'clock, in an appropriate container.
All urine samples must be returned with the form below. The form can be printed off, emailed or requested at the surgery. If you are unable to print/email the form you can supply a note with your sample as long as you have answered all questions on the form and given the practice your details.
