Before filling out a registration form, check you are in our catchment area opposite.

To register

To register with us, please complete our registartion form, along with the Health questionnaire.  These forms will need to be returned to the surgery with proof of your address. We are not able to register you with the practice until the forms have been completed.  Copies can be downloaded from the links below or hard copies from our reception desk.

Some examples of proof of address accepted by the practice are:

  • Utility Bills
  • Council Tax
  • Tenancy Agreement

Non-English Speakers

The Practice has access to an interpreter through Lanaguage Line.  Please inform our reception team if you require access to this service when booking an appointment.

Please complete the registration form, along with the corresponding Health questionnaire for each member of the household you wish to register with the practice.

All forms need to be completed, signed and returned to Pontprennau Medical Centre in order for you to be registered with the practice.

In order to register you we ask that you fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. 

Temporary Patient Registrations

 

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
