How to get your medical records

Medical records hold information about you. They are also sometimes called health records.

You'll have separate records for any NHS service you go to including your GP surgery, hospital, dentist or opticians.

How to get your record depends on which record it is.

How to Request a copy of your GP record

Your GP records include details such as any medical conditions, allergies, and medications you are currently taking.

Under GDPR regulations, we are required to protect your data. This includes ensuring we have your consent to release copies of your information and maintaining a record of all requests. We must also redact any third-party data before providing access.

If you wish to obtain a copy of your medical records, those of your child (if they are under 13), or for someone you have lasting power of attorney for, please complete and return the relevant form. We cannot provide data for anyone else without their explicit consent. Please note that having permission to speak on behalf of another patient does not automatically grant permission to access their data.

You can submit your completed form via email at reception.pontprennau@wales.nhs.uk, send it by post, or return it in person to reception. Requests are processed within 30 days of receipt and are free of charge, but duplicate requests may incur a fee.

Once your requested information is ready, we will notify you by email or SMS. If neither is available, we will call you. Please note that documents will only be held for one month. If not collected within this period, they will be securely destroyed.
